Paediatric caudal anaesthesia
نویسنده
چکیده
Caudal anaesthesia (CA) is epidural anaesthesia of the cauda equina roots in the sacral canal, accessed through the sacral hiatus. CA is a common paediatric regional technique that is quick to learn and easy to perform, with high success and low complication rates. CA provides high quality intraoperative and early postoperative analgesia for sub-umbilical surgery. In children, CA is most effectively used as adjunct to general anaesthesia and has an opioid-sparing effect, permitting faster and smoother emergence from anaesthesia. INDICATIONS FOR CAUDAL ANAESTHESIA The indications for single shot CA are abdominal, urologic or orthopaedic surgical procedures located in the sub-umbilical abdominal, pelvic and genital areas, or the lower limbs, where postoperative pain does not require prolonged strong analgesia. Examples of appropriate surgery include inguinal or umbilical herniorrhaphy, orchidopexy, hypospadias and club foot surgery. CA is useful for day case surgery, but opioid additives to the local anesthetic agent should be avoided in this setting. When CA is used, requirement for mild or intermediate systemic analgesia must be anticipated to prevent pain resurgence at the end of caudal block. Catheter insertion can extend the indications to include surgical procedures located in the high abdominal or thoracic areas, and to those requiring prolonged effective analgesia.

متن کاملCaudal blocks.
Caudal anesthesia is the single most important pediatric regional anesthetic technique. The technique is relatively easy to learn (1), has a remarkable safety record (2), and can be used for a large variety of procedures. The technique has been reviewed in the English (3) and French (4) literature, as well as in German guidelines (5) and in pediatric anesthesia textbooks (6).

BACKGROUND AND OBJECTIVES Emergence agitation is a common postanaesthetic problem in children after sevoflurane anaesthesia. We aimed to compare the effects of ketamine and midazolam administered intravenously, before the end of surgery, for prevention of emergence agitation in children who received caudal block for pain relief under sevoflurane anaesthesia. METHODS 62 American Society of Ane...

Introduction: We previously reported a case using caudal morphine for pediatric liver transplantation. A 3-yr-old, 14.9 kg child received a caudal injection of 0.6 mg of preservative free morphine sulfate as well as 110 mcg of intravenous fentanyl. Also, the surgical incisions were infiltrated with 20 ml of 0.125% bupivacaine1. However, a question arose as to the impact of the local anesthetic ...
متن کاملDelayed vomiting after papavaretum in paediatric outpatient surgery.
The contribution of papavaretum to the incidence of vomiting observed in the 24 hours after paediatric outpatient anaesthesia was assessed in 129 children undergoing circumcision. Postoperative analgesia in all patients was achieved using caudal extradural blockade. The incidence of vomiting in the 24 hours after discharge from hospital was 56 per cent in those receiving papavaretum, compared w...
